Priority Placement Verification Information Form:  FMP 2606  

Central Registration controls the waiting lists for all childcare services, including Family Child Care and  the center-based programs. There are three Child Development Centers, one Youth Center and numerous family child care homes on Andrews AFB. The School-Age Programs (Before and After School and Camp Programs) are located at the Youth Center. 

Request for Child Care, DD Form 2606, is used in requesting childcare. These forms are categorized as follows: Unmet Need, Projected Demand and Preferred Care. Then they are prioritized according to the base priority plan.   

Eligible patrons are active duty military, 
Department of Defense (DoD) civilians’ personnel, 
Reservists on active duty and DoD contractors. 
The installation commander has established the following priority system:

IAW AF 34-701, Child Care Program, Paragraph 7.9 and is approved for use by the Family Member Programs Flight located on Andrews Air Force Base.


1. The following applies to those assigned to AAFB:
Single/Dual Active Duty Military; Single/Dual DoD Civilian Employees; Active Duty Military with working spouse; DoD Civilian employees with working spouse


2. The following applies to those not assigned to AAFB:
Single/Dual Active Duty Military; Single/Dual DoD Civilian Employees; Active Duty Military with working spouse; DoD Civilian Employees with working spouse


3. The following applies to those assigned to AAFB:
Active Duty Military with non-working spouse; DoD Civilian Employees with non-working spouse


4. The following applies to those not assigned to AAFB:
Active Duty Military with non-working spouse; DoD Civilian employee with non-working spouse
- Assigned to AAFB means: Sponsor must be working or living on AAFB Base, living in the Summerfield Housing Area, working at a detachment that is home based at AAFB.
-Working spouse means: Having a job outside the home or a home based business or spouses who are FULL Time Students (12 credit hours being taken.)

Demand

UNMET childcare is for families who desire care on Andrews AFB and do not have any form of care on the base. Within this level families are prioritized according to the base priority policy. Priority One are those single/dual military and military with a working spouse, DoD employees single or with working spouse who are directly living on AAFB or in the Summerfield Housing Area and/or working on AAFB. Other priorities are placed on the list, and may be offered care in the FCC homes or in one of the center base programs, however, if a waiting list develops then they will be give written notice of termination to accommodate the priority one need. 

Preferred Waiting List
Since our waiting list feeds into both the Family Child Care homes and center based programs, family may accept a program as an alternative to their first desire.  For example placed in a home and prefer a center based program, or placed in a particular center but prefers a particular center. When this happens the sponsor may request a transfer to a specific program or specific location for care. Before this transfer will be approved there must be an opening and the unmet requests for care have to be accomplished. If there is an emergency request which is fully justified it will be handled on a case-by-case basis.
Projected Demand
Projected Demand is the third type of waiting list category.  
There are three levels to this program, the unborn child, the families coming to AAFB under PCS orders and any family who is in the process of adoption. In order to be placed on the UNBORN list a medical verification of pregnancy must be presented then the DD Form 2606 may be filled out and filed with SPS. The application is date stamped and filed alphabetically until notification of the baby’s arrival. When sponsor receives orders to PCS to Andrews they may file the DD Form 2606 with SPS. A copy of the inbound orders must accompany the request with an anticipated date of arrival. Once the baby is born or the family arrives on AAFB they are to notify SPS and update any information which has changed then the application will be properly filed in the Unmet Listing according to the original date on the application.
Registration
When called  for child care and the offer is accepted all registration paperwork is accomplished by appointment at the Central Registration office. In order to complete registration paperwork, parents will need child's shot record, current LES/earnings statement plus names and phone numbers for local emergency contacts. Once this is accomplished a mandatory orientation is scheduled for the receiving program. 

The following MUST be presented at the Enrollment Appointment:

Single/Dual Military: 
AF Form 357 (Dependant Care Plan) or
Equivalent form from your branch of the military
or a Notarized statement of your Dependant Care Plan from Judge Advocate’s Office.  Failure to bring this document to the Central Registration Office prior within 48 hours of enrollment appointment will result in your records not being forwarded to the center. Care will be denied resulting in the offer being withdrawn and your application will returned to the bottom of the waiting list. 

All Applicants:

Children’s Shot Records:
If records indicate that shots are missing the Immunization Clinic will be contacted to confirm the necessity of the shot. If it is determined that the child is delinquent in having the proper inoculations, you will be directed to contact the Immunization Clinic and develop a plan for the shots to be caught up to date. This plan must be turned into the Central Registration Office 1 (One) week prior to the start date of enrollment. If not presented, enrollment will be suspended until this plan is in-place. Parents will be responsible of all fees charged for care from the start date forward.

Leave & Earning Statement/ Salary Verification
All military applicants must present their most recent 1st of the month Leave & Earning Statement. Civilians must present their most recent pay statement. If new employment is the case letter verification from employer must be presented to indicate yearly salary, monthly salary, hourly wage with anticipated number of hours worked per week. If these items are not available the fees for the care will be charged at the highest rate allowed. Once the fees have been determined they will not be adjusted until the beginning of the next fiscal year. If statements have not been provided then sponsor may apply for a fee reduction subject to approval by the Services Squadron Commander.

Part-Day Enrichment Program

Part-Day Enrichment Program is designed for three and four year old children who are not in need of center based full day care. The main focus in the curriculum is the social skills needed for a successful life and offers children an excellent opportunity to work and play with other children in preparation for school. 

During the three-hour program the children choose their own activities, participate in learning activities.

The children are read to every day and their language is put into words on charts for them to read. The children play outdoors daily, weather permitting and in inclement weather, they have access to a gym. A healthy snack is also included as part of the program and the staff assist children in learning self help skills of setting the table, pouring their milk and passing plates around the table.  We consider the parents as partners in their child's development and maintain an open door policy for parents to visit and share their talents and experiences. Educational field trips are included as part of the curriculum at no extra charge. Children in the Part Day Enrichment Program develop a love of learning that will follow them all of their lives. 

The classes are offered in three different configurations. You may choose from a 2-day, 3-day or 5-day program. The 2-day program meets on Tuesday and Thursday morning form 0845-1145. The 3-day program meets on Monday, Wednesday and Friday from 1100-1400. Currently if you prefer a 5-day experience you will combine the 2 and 3-day program.
